• Age >65 years

• Immigrants from endemic countries

• Alcoholics and drug abusers

• HIV-positive patients

• Immunosuppressed patients

• Patients treated with immunosuppressive therapy including biologics (tumor necrosis factor [TNF] antagonists > others)

Glucocorticoids at prednisone dose ≥10 mg/day significantly adds to these risk factors.
