How does TB disseminate to the bone and joint?
• Hematogenous spread during primary infection.
• Lymphatic spread from distant focus.
• Contiguous spread from infected areas.
Although joint involvement may be secondary to hematogenous spread, it usually is secondary to an adjacent osteomyelitis. Therefore, tuberculous arthritis is usually a combination of bone and joint involvement.
