What is Isoproterenol
Isoproterenol is a parenteral non-selective beta-adrenergic agonist indicated for mild or transient episodes of heart block that do not require electric shock or pacemaker therapy, serious episodes of heart block and Adams-Stokes attacks (except when caused by ventricular tachycardia or fibrillation), cardiac arrest until electric shock or pacemaker therapy is available, bronchospasm occurring during anesthesia, and as an adjunct to fluid and electrolyte replacement therapy and other medications and procedures in the treatment of hypovolemic and septic shock, low cardiac output states, congestive heart failure, and cardiogenic shock.
It is a synthetic sympathomimetic amine structurally similar to epinephrine. Isoproterenol has very low affinity for alpha-adrenergic receptors.
Guidelines do not recommend isoproterenol for use during cardiac arrest.
Although not a first line agent, isoproterenol may be used for bradyarrhythmias unresponsive to or inappropriate for treatment with atropine or while awaiting the availability of a pacemaker
In patients with sinus node dysfunction or second- or third-degree atrioventricular block associated with symptoms of hemodynamic compromise who are at a low likelihood of coronary ischemia, isoproterenol may be considered to increase heart rate, improve atrioventricular conduction, increase ventricular rate, and improve symptoms.
