What is Codeine Guaifenesin

Codeine guaifenesin are used together in oral preparations to relieve coughing due to minor throat and bronchial irritation as may occur with the common cold or inhaled irritants.

Codeine is an opiate agonist used as an antitussive agent. Codeine has a drying effect on respiratory tract mucosa and increases viscosity of bronchial secretions. Codeine also has mild analgesic and sedative effects. Guaifenesin loosens and thins phlegm and bronchial secretions to ease expectoration. Despite its long history of use since the 1950s, the efficacy of guaifenesin has only recently been substantiated.

Although guaifenesin may be used as an expectorant in both productive and non-productive coughs, it is especially useful in the treatment of dry, non-productive coughs which tend to injure the respiratory mucous membranes.

Codeine is a prodrug that is hepatically metabolized to morphine. Codeine metabolism is highly variable and unpredictable, particularly in children younger than 12 years; therapeutic response to recommended doses can range from lack of effect in poor metabolizers to fatality in ultra-rapid metabolizers.

Opioid cough and cold medicines that contain codeine are not indicated in pediatric patients under 18 years of age.

These products are contraindicated in neonates, infants and children less than 12 years of age; risks of opioid-containing cough and cold medicines outweigh their benefits in patients younger than 18 years.

Codeine guaifenesin is also contraindicated in adolescent pediatric patients 12 to 18 years of age after tonsillectomy or adenoidectomy.
