Congenital adrenal hyperplasia is a diverse family of autosomal recessive disorders characterized by deficient function of one of the enzymes necessary for cortisol synthesis.

=The reduction in cortisol synthesis leads to a loss of feedback inhibition of the hypothalamic-pituitary-adrenal axis, with excessive production of corticotropin.

Exposure to excessive corticotropin leads to adrenal hyperplasia, overproduction of adrenal steroids that do not require the deficient enzyme, and deficiency of steroids distal to the deficient enzyme.
