Another prophylactic treatment has been N-acetylcysteine (also known as Mucomyst), which has antioxidative properties by increasing intracellular glutathione levels. It is believed that N-acetylcysteine may exert its activity by negating some of the effects of the reactive oxygen species. In prospective studies, N-acetylcysteine is not consistently effective at reducing the risk of contrast nephropathy. The PRESERVE Trial referenced in the Prophylactic Hydration section also randomized patients at risk for CIN to N-acetylcysteine or placebo and found no prophylactic benefit of N-acetylcysteine. Based on these results, we do not recommend N-acetylcysteine prophylaxis.
