OPLL stands for ossification of the posterior longitudinal ligament and typically involves the cervical spine. OPLL appears as a longitudinal bony ridge parallel and adjacent to the posterior vertebral margins.

It is more common among individuals of Japanese descent and occurs in 50% of patients with diffuse idiopathic skeletal hyperostosis (DISH). OPLL can result in spinal canal stenosis.
