An IACB is often used in critically ill patients with left ventricular failure, unstable angina, acute myocardial ischemia/infarction associated with percutaneous transluminal angioplasty, or severe mitral regurgitation. Major contraindications to IACB placement include severe aortic insufficiency, aortic dissection, severe peripheral vascular disease, or contraindication to systemic anticoagulation.
