  • Progression over longer than 8 weeks or a relapsing course
  • Symmetric proximal and distal weakness in all extremities (legs more than arms)
  • Sensory impairment
  • Hyporeflexia or areflexia in all extremities
  • Elevated cerebrospinal fluid (CSF) protein without pleocytosis (cytoalbuminologic dissociation)
  • Electrodiagnostic evidence of demyelinating neuropathy
  • Pathologic evidence of demyelinating neuropathy on sural nerve biopsy
