What is the vascular anatomy of the pancreatic transplant?

The donor SMA and splenic artery are anastomosed to a donor iliac artery Y-graft (composed of the common iliac artery and the proximal internal and external iliac arteries). The donor common iliac artery is then anastomosed to the recipient common iliac artery or external iliac artery. The SMA supplies the head of the pancreatic transplant, whereas the splenic artery supplies the body and tail of the pancreatic transplant. The donor portal vein is attached to the recipient superior mesenteric vein (SMV), iliac vein, or IVC and receives venous blood flow from the donor SMV and splenic vein.
