Copper deficiency myelopathy usually presents with a progressive gait abnormality due to both dorsal column dysfunction with a sensory level, as well as corticospinal tract dysfunction with spastic para or tetraparesis.

The clinical picture can sometimes be confused from a coexisting sensory neuropathy with decreased distal reflexes.
