Prognosis of HSP nephritis

What is the prognosis of HSP nephritis?

Most patients have a self-limiting course and a good prognosis.

A minority of patients have persistent hematuria or proteinuria that may eventually lead to end-stage kidney disease.

In these patients, the evolution and prognosis of the nephritis is comparable to that of patients with IgAN; thus the follow-up and approach to treatment should be similar.

There is limited evidence to guide treatment, but steroids with or without other immunosuppressive agents may be a reasonable option for severe and progressive kidney disease.

Recurrent/relapsing episodes can occur but may not predict a worse long-term outcome. Disease behavior after transplantation is similar to that of IgAN but occasionally can be severe, with features of rapidly progressive GN and crescentic disease.
