Imaging studies are indicated if red flags (see Question 12) are present because the yield is increased if you suspect cancer, fracture, infection, or inflammatory spondyloarthropathy. In patients with chronic back pain (>12 weeks) despite appropriate therapy for acute lumbar strain, spine films should be considered.
