  • • If no active spinal lesions: NSAIDs initially, although this is not sufficient for the majority of patients.
  • • If refractory to NSAIDs or with active spinal lesions: nonbiologic disease-modifying antirheumatic drugs (methotrexate or sulfasalazine), TNF-inhibitors, and/or bisphosphonates
