  1. Traumatic (acute or repetitive) hypertrophy of the richly innervated infrapatellar fat pad
  2. Differentiated by:
    • Pain is mainly subpatellar
    • Hoffa sign: if pain is elicited when clinician puts pressure on patellar tendon margins as patient extends bent knee, Hoffa syndrome is suggested
  3. Plain radiographs usually show normal findings; MRI may be useful for diagnosis
