Tizanidine is an alpha 2 agonist agent that has been prescribed for spasticity, as an analgesic and as a muscle relaxant for many years. Multiple studies have suggested its benefit for spasticity, low back pain, fibromyalgia, neuropathic pain, and headache. One might consider capitalizing on its sedating effect by dosing initially at night—typically a 2-mg starting dose at bedtime may be well-tolerated. In many instances, titrating every 3 to 5 days to 4 to 8 mg at bedtime may be effective. Daytime doses are not always tolerated due to its sedating effect. Sedation, dizziness, and dry mouth are the three most common side effects, and liver functions should be periodically monitored if used on a long-term basis.
