What is Fenfluramine

Fenfluramine is an oral 5HT-2 receptor agonist indicated for the treatment of seizures associated with Dravet syndrome.

Fenfluramine significantly reduced the monthly convulsive seizure frequency compared to placebo in 2 randomized, controlled trials in pediatric patients (2 to 18 years) with Dravet syndrome.

In a randomized, double-blind trial of patients with Dravet syndrome whose seizures were not adequately controlled on 1 or more antiepileptic drugs (n = 119), the median reduction in seizure frequency was 42.3% and 74.9% in patients treated with adjunctive fenfluramine 0.2 mg/kg/day and 0.7 mg/kg/day, respectively, compared to 19.2% in patients given placebo. Use of fenfluramine 0.2 mg/kg/day resulted in a 32.4% greater reduction in mean monthly convulsive seizure frequency when compared to placebo (95% CI 6.2% to 52.3%, p = 0.0209); fenfluramine 0.7 mg/kg/day was associated with a 62.3% greater reduction in mean monthly convulsive seizure frequency vs. placebo (95% CI 47.7% to 72.8%; p less than 0.0001).

Patients treated with fenfluramine 0.4 mg/kg/day as part of a stable antiepileptic drug regimen containing stiripentol (n = 87) experienced a 54% greater reduction in mean monthly convulsive seizure frequency compared to placebo (95% CI 35.6% to 67.2%; p less than 0.001).

Most patients responded to treatment with fenfluramine within 3 to 4 weeks, and the effect generally remained consistent over the 14- or 15-week treatment period.

Fenfluramine was previously FDA-approved for the treatment of obesity; however, due to cases of valvular heart disease attributed to fenfluramine, it was voluntarily withdrawn from the market for this use.

In patients with Dravet syndrome, echocardiogram monitoring before treatment, every 6 months during treatment, and 3 to 6 months after treatment with fenfluramine is required to evaluate for valvular heart disease and pulmonary hypertension.
