Are long acting opioid preparations preferred over short acting types?

Theoretically, extended release, long-acting (ER/LA) opioids afford cancer pain relief over a longer period of time (8 to 72 hours based on the product properties), whereas the duration of action of immediate release (IR) opioids is limited to about 4 to 6 hours. There is little evidence that ER/LA opioids provide pain relief that is superior to IR opioids, but it must be stressed that all pain patients benefit from scheduled, around-the-clock dosing schedules, as opposed to their use on an as-needed (prn) basis.
