Why is the use of radiation therapy in initial treatment of primary CNS lymphoma controversial? 

Despite primary CNS lymphoma being extremely sensitive to radiation therapy, it is often withheld due to high rates of cognitive problems especially in elderly patients.

For patients who are not candidates for high-dose systemic methotrexate (i.e., renal failure), it has continued to be an option for palliative treatment.

Additionally for younger patients with a good cognitive performance status, radiation therapy after initial methotrexate therapy may be beneficial and continues to be further studied.
