Why is MRI important in the early diagnosis of occult proximal femoral fractures?
MRI is the most sensitive and specific method for early diagnosis of radiographically occult fractures because its superior contrast resolution allows for visualization of the linear fracture signal abnormality and the associated bone marrow edema immediately after the fracture has occurred. If there is a strong clinical suspicion of an occult fracture, for example, in an osteoporotic patient with hip pain, MRI should be performed for early diagnosis.
Delayed diagnosis and treatment of these injuries can lead to complications such as AVN, progression from incomplete to complete fracture, fracture displacement, and arthritis, which would require more invasive treatment methods, including hip replacement.
