What is the relationship between hypertension and hyperparathyroidism?

Hypertension occurs in a variable proportion of patients with primary hyperparathyroidism, depending on the population surveyed; “cure” of hypertension is similarly unpredictable after resection of the parathyroid adenoma. Oddly enough, other chronic conditions that produce similar levels of hypercalcemia (e.g., sarcoidosis, myeloma, or other malignancies) are not associated with hypertension. This has led to increased attention to the serum parathyroid hypertensive factor, a unique 3000 to 4000 Da protein, which is normally released from parathyroid glands, and raises blood pressure when infused into rats or humans. Although its plasma concentration can now be measured by radioimmunoassay, it is not routinely measured in clinical laboratories.
