Parameters to be monitored in progressive kidney disease
What are the parameters that should be monitored in bone and mineral metabolism, and when are changes in these parameters likely to occur in the person with progressive kidney disease?
The parameters that should be monitored in patients with progressive kidney disease with respect to bone and mineral metabolism include serum levels of calcium, phosphorous, 25-hydroxyvitamin D, and intact parathyroid hormone (PTH).
Although phosphorous retention occurs early in CKD, a rise in PTH (particularly when GFR < 60) serves to maintain serum calcium and serum phosphorous within normal ranges in the vast majority of patients until late stages of progressive kidney disease.
