Most common non iatrogenic cause of pneumobilia?
Gas in the biliary tree, or pneumobilia, usually results from communication of bowel with the biliary tree, not a gas-forming infection. The most common cause of pneumobilia is probably endoscopically or surgically performed sphincterotomy or a choledochoenteric anastomosis. In patients who have not undergone iatrogenic interventions, the most common cause of pneumobilia is a penetrating duodenal ulcer. Other etiologic factors include choledochoduodenal or cholecystoduodenal fistula owing to gallstones eroding into the duodenum.
