- 1. Intractable pain that has been refractory to conservative therapy
- 2. Severe or progressive motor weakness or sensory deficits
- 3. Symptoms refractory to a reasonable period of nonoperative therapy
- 4. Of note, outcomes are much better when signs and symptoms correlate with radiographic findings.
