The most common causes are surgery and instrumentation. Pneumoperitoneum can be found after laparotomy/open surgery, paracentesis, or resuscitation. Also, distal bowel obstruction from conditions such as Hirschsprung’s disease or meconium ileus and dissection of gas from pneumomediastinum, a ruptured ulcer, or Meckel’s diverticulum can lead to pneumoperitoneum.
