Meperidine and Promethazine

What is Meperidine and Promethazine

Meperidine and promethazine are used together in an oral preparation to treat pain. Meperidine hydrochloride is a synthetic opiate agonist belonging to the phenylpiperidine class.

Other members of the phenylpiperidine group include alfentanil, diphenoxylate, fentanyl, loperamide, and sufentanil. The chemical structure of meperidine is similar to local anesthetics.

Meperidine is recommended for relief of moderate to severe acute pain and has the unique ability to interrupt postoperative shivering and shaking chills induced by amphotericin B.

Meperidine is considered a second-line agent for the treatment of acute pain. Promethazine is a phenothiazine derivative; however, it is not used clinically as a neuroleptic.

Promethazine exhibits significant antihistaminic (H1-blocker) activity with considerable anticholinergic, sedative, and antiemetic effects and some local anesthetic properties.

In November 2004, the FDA directed manufacturers of promethazine-containing products to include a boxed warning contraindicating use in children and infants less than 2 years of age due to an increased risk for fatal respiratory depression.

Meperidine and promethazine products should be used with considerable caution in pediatric patients 2 years of age and older.

Meperidine and promethazine combination products were originally approved by the FDA in November 1961.
