Palliative care interdisciplinary team

Who are the members of a palliative care interdisciplinary team?

Palliative care interdisciplinary team (IDT) members include a medical director (palliative care physician), registered nurses, social workers, psychologists, pastoral care counselors, volunteer coordinators, and others who assist with formulating and delivering the plan of care to patients receiving palliative or hospice care. The IDT meets regularly to discuss the patients under their care, and assess pain and symptom burden, as well as psychosocial concerns; the team can intervene at many levels to support that patient with cancer and his family and caregivers. The goal is to provide comprehensive, supportive care, as well as to utilize appropriate pharmacologic, nonpharmacologic, or CAM treatments.
