Fractures are very common following kidney transplantation associated with bone loss in the first 1 to 2 years.

The use of corticosteroids for immunosuppression is considered to be the major contributor. The decreased use of corticosteroids has resulted in less fractures following transplantation.

However, other factors such as persistent hyperparathyroidism, vitamin D deficiency, persistent and/or progressive CKD, and hypophosphatemia are contributing factors.

Therapy should be focused on identifying the underlying metabolic disorder and appropriately managing it.
