In patients with prior history of aHUS, the disease recurs in approximately 50% of patients who are transplanted. Graft failure occurs in 80% to 90% of patients with recurrent aHUS.

Living related renal transplantation is contraindicated in aHUS even if genetic screening is negative, because of the risk of unidentified genetic susceptibility factors.

Prophylactic treatment with eculizimab has been discussed as a therapeutic option.
