When NSCLC is stage IIIB (i.e., with the presence of invasion of vital mediastinal structures or unresectable lymphadenopathy) or stage IV (i.e., with the presence of metastases involving the pleura, contralateral lung, or distant sites), it is generally considered unresectable. Symptoms and signs are rarely present until the disease is advanced in stage and more likely to be unresectable.
