Pseudohypercalcemia

In pseudohypercalcemia the ionized calcium is normal, but the total serum calcium is elevated due to increased calcium binding to serum proteins.

This can be seen in patients with hyperalbuminemia due to dehydration.

Also, in certain multiple myeloma cases, the abnormal paraprotein can bind excessively to calcium, resulting in elevated total serum calcium.
