Can vascular dementia be diagnosed by CT or MRI alone? 

No. Patients who have changes in white matter on scans or even multiple, definite infarctions may be clinically normal.

It is not known how many infarcts or how much change in white matter on a scan translates into dementia for patients who suffer cognitive impairments.

Many white matter signal changes, especially on MRI, do not represent strokes.
