Most infants tolerate standard infant formulas made with either intact cow’s milk or soy milk protein.

However, situations arise that require use of an alternative specialized formula, such as persistent feeding intolerance due to minor discomfort (i.e., fussiness, excessive gas, or crying), gastroesophageal reflux (GER), allergies, and/or malabsorption.

Infants may present with a diverse range of symptoms that are triggered by specific food proteins or other food constituents.

Because of the diverse spectrum of presenting symptoms and spectrum of diseases, it is often difficult to define an exact etiology.

Furthermore, because dietary factors directly influence gut functionality and motility, empirical treatment with dietary macronutrient manipulation is a common practice and often confers symptom reduction.

Infant formulas for feeding intolerance, allergies, and/or GER are nutritionally balanced; when mixed to standard concentration, these formulas contain approximately 20 kcal/oz.

These specialized formulas can be broken down into 3 main categories: partially hydrolyzed protein formulas for minor feeding intolerance, extensively hydrolyzed protein or amino-acid based formulas for allergies, malabsorption, and maldigestion, and thickened formulas for infants with GER.
